"Canadian scientists have developed a blood test and portable device that can determine the onset of sepsis faster and more accurately than existing methods.
Published today [May 27, 2025] in Nature Communications, the test is more than 90 per cent accurate at identifying those at high risk of developing sepsis and represents a major milestone in the way doctors will evaluate and treat sepsis.
“Sepsis accounts for roughly 20 per cent of all global deaths,” said lead author Dr. Claudia dos Santos, a critical care physician and scientist at St. Michael’s Hospital. “Our test could be a powerful game changer, allowing physicians to quickly identify and treat patients before they begin to rapidly deteriorate.”
Sepsis is the body’s extreme reaction to an infection, causing the immune system to start attacking one’s own organs and tissues. It can lead to organ failure and death if not treated quickly. Predicting sepsis is difficult: early symptoms are non-specific, and current tests can take up to 18 hours and require specialized labs. This delay before treatment increases the chance of death by nearly eight per cent per hour.
[Note: The up to 18 hour testing window for sepsis is a huge cause of sepsis-related mortality, because septic shock can kill in as little as 12 hours, long before the tests are even done.]
[Analytical] AI helps predict sepsis
Examining blood samples from more than 3,000 hospital patients with suspected sepsis, researchers from UBC and Sepset, a UBC spin-off biotechnology company, used machine learning to identify a six-gene expression signature “Sepset” that predicted sepsis nine times out of 10, and well before a formal diagnosis. With 248 additional blood samples using RT-PCR, (Reverse Transcription Polymerase Chain Reaction), a common hospital laboratory technique, the test was 94 per cent accurate in detecting early-stage sepsis in patients whose condition was about to worsen.
“This demonstrates the immense value of AI in analyzing extremely complex data to identify the important genes for predicting sepsis and writing an algorithm that predicts sepsis risk with high accuracy,” said co-author Dr. Bob Hancock, UBC professor of microbiology and immunology and CEO of Sepset.
Bringing the test to point of care
To bring the test closer to the bedside, the National Research Council of Canada (NRC) developed a portable device they called PowerBlade that uses a drop of blood and an automated sequence of steps to efficiently detect sepsis. Tested with 30 patients, the device was 92 per cent accurate in identifying patients at high risk of sepsis and 89 per cent accurate in ruling out those not at risk.
“PowerBlade delivered results in under three hours. Such a device can make treatment possible wherever a patient may be, including in the emergency room or remote health care units,” said Dr. Hancock.
“By combining cutting-edge microfluidic research with interdisciplinary collaboration across engineering, biology, and medicine, the Centre for Research and Applications in Fluidic Technologies (CRAFT) enables rapid, portable, and accessible testing solutions,” said co-author Dr. Teodor Veres, of the NRC’s Medical Devices Research Centre and CRAFT co-director. CRAFT, a joint venture between the University of Toronto, Unity Health Toronto and the NRC, accelerates the development of innovative devices that can bring high-quality diagnostics to the point of care.
Dr. Hancock’s team, including UBC research associate and co-author Dr. Evan Haney, has also started commercial development of the Sepset signature. “These tests detect the early warnings of sepsis, allowing physicians to act quickly to treat the patient, rather than waiting until the damage is done,” said Dr. Haney."

Android-X, the Lord of All that is Metal. Formerly Tap, Emperor of Agartha, Android-X is the Overlord of the Supreme Technate of Neo-Agartha, an expansionist, militaristic nation of androids and robots all made from living metal, and powered by it's reality-warping abilities. They are an evil genius, focused on WORLD DOMINATION in the name of progress, unity, and equality for all.
Unfortunately, they're ya'know kind of dead so, no more them.
Viliness grew from a glob of ooze that slithered over to the pantry and lived off candy, pastries, and sacks of sugar for much of their early life. They are made mostly out of sugar and molasses and are more human-shaped than the rest with an oversized flexible tail that grows at a steady, constant rate. They energetic, childish, and are a big eater. After their father became Android-X, they struck it out to become a freedom fighter and revolutionary, fighting against the Metal Army with cryo and plasma.
Sterling grew from one that climbed down, down, down and spent their youth eating scraps of living metal in the foundry. They are a silvery-grey four-armed creature with malformed pieces of machinery floating about in their mass and the ability to shapeshift parts of their body into machinery, such as drills, guns, and propellers. They are very reasonable and pragmatic, but also marshal and military-minded, often practicing their hand-to-hand against robots. is weak to electricity. They remain loyal to their father and have learned to harness their power to bend reality, and now acts as the Prime General of the Metal Army. To them, the flesh is weak, emotions are flaws, and efficiency is god.
Salz grew from one that made it's way to the chemistry lab and ate chemicals and elemental salts for it's early days. Now, they are sort of wolflike in bodyshape and very grainy, made as much of salt crystals as ooze, and are an expert chemist, particularly in the field of bioinorganic chemistry, though they are a nervous, cautious fellow. constantly flaking off pieces. They now live on the same refugee dragon-planet as their sister Viliness, having delved into eldritch alchemy and biotechnology.
And Lewry grew from one that ended up in the library and would eat the pages of arcane and occult tomes. They float off the ground like they're suspended in water, with their legs reduced to flippers and a tail like an eel. They are grey in color and have all sorts of natural arcane powers, such as spewing hellfire, telekinesis, phasing through objects, ETC. Personality-wise, they are kind of the trickster of the group, fond of sneaking up and spooking people, and really good at spotting sneaks themself. Can't physically interact with non-magical substances. After the robo-revolution, they slithered into the digital dimension of cyberspace and became a revolutionary.
Warren Worlds is a lunarian (a kind of cave-dwelling mothman) refugee from old Agartha from an old money family who has since fled to the Dragon-planet with the others. They have a glossy black exoskeleton, wine-red eyes, and wings with patterns of inky black and frosty white. They are a professor of civil engineering in Menoa's (from @serious-tabaxi) campus, at which they do fairly well. They are very knowledgeable in all manner of subjects, but lack any practical knowledge, is fairly casual, but short tempered and holds grudges. They have no dark secrets. none at all. zip. zero. they are CLEAN as SOAP 100%. They suffer from headaches often.
Rattler is a rattlesnake naga, about 45 feet long and as thick as a man's torso, who lives in a sandstone cave in a rocky desert. They are rather vain, considering themself the height of beauty, and dismissive of "warm-bloods", which they mostly just see as food or sycophants. They've picked up some skills in elementalist magic, which they use to expand their desert, desiccating the soil and growing pillars of pyromantic glass to maintain the heat. Their den is staffed by creatures of the desert, like giant scorpions, earth elementals, kobolds, and cultists, many of which know a touch of necromancy and can in turn, add skeletons and mummies to their roster. They have no real ambition or ill will, just a belief in the superiority of reptiles over mammals and a desire to terraform the world to be more to their liking.

June Lore Challenge: Day 8

Today is some simple but interesting lore about the SeeKeR Project, which was done by the Obscorian World Government back in the early days. The SeeKeR Project was a biotech experiment done on a creature known as the Seagrass Iguanosaurus. 14 generations of the SeeKeRs were created, the first 13 being almost total failures.
Biotech and Nanites
Biotechnology typically is used in prosthetics to use the body’s natural electrical signals to control a robotic part. For the SeeKeR project, scientists experimented with a special type of biotech which uses microscopic robots. The robots are called Nanites. Nanites are able to create artificial structures in the body before expiring. Dead Nanites are excreted from the body in an organism’s poop.
Nanites have some radiation output due to being powered by ignitium dust, so there is a limit to how many can be used before causing damage to an organism’s organic cells and tissue. They are usually used as a last resort in a medical setting. Nanites can have a variety of different uses in medicine, including reconstructing flesh.
The SeeKeRs
SeeKeRs were created to assist the Unity Police in tracking down wanted criminals by using a facial recognition technology. To create these new biotech animals, they injected Seagrass Iguanosaur eggs with specifically coded nanites which would cause the Iguanosaurs to grow necessary biotechnological parts. Unfortunately, many of the first generations died due to tumors that developed as a result of the nanite’s radiation output. One model, known as the SKR-12, developed some sort of genetic mutation from the nanite’s meddling which caused neurological issues. Scientists returned one morning to find 12 had killed and eaten the biological flesh of SKR-10 and SKR-6. SKR-12 was put down and generation 13 was started. 13 also suffered from neurological issues and tried to remove the green visor that had replaced its face in the night. It was given an emotional support dog to keep it from hurting itself, and it was decided that no more SKR-13 models would be created due to their emotional instability. SKR-14 was a success, and was sent out the following month with instructions to hunt down criminals. However, it was quickly discovered that the AI used to detect faces was incredibly flawed, and would mark innocent people as criminals based entirely on their facial structures being similar to wanted criminals. The Government had the SKR-14 retired and the AI was shut off, but the creatures had to be released back into the wild due to animal welfare concerns. The SKR-14s became an invasive species, the biotech pieces now a part of their genetic code, and can be found all over Obscoria.

Objective:
To share an intriguing case of a young transgender male patient receiving testosterone therapy who developed locked-in syndrome due to an acute ischemic stroke and to highlight potential risk factors for stroke in the LGBTQI+ community
Background:
There are many studies identifying risk factors for stroke in racially and ethnically diverse populations. However, there is little existing data for stroke risk factors in the LGBTQI+ community. Prior research has shown testosterone therapy in cis-gender men with initially low levels of testosterone increases the risk of stroke, especially in the first 2 years of use1. While testosterone therapy has been shown to increase the risk of venous thrombosis, its role in arterial thrombosis is unclear2. A proposed mechanism for thrombosis with testosterone replacement includes erythrocytosis, but the potential contribution of an independent pro-coagulant effect is yet to be determined3.
Design/Methods:
Literature review and case report.
Results:
An otherwise healthy 23-year-old transgender male on one year of testosterone therapy presented in an obtunded state. Examination revealed complete quadriplegia with sparing of vertical eye movements, consistent with locked-in syndrome. Imaging revealed complete occlusion of the basilar artery with distal reconstitution at the superior cerebellar arteries, and a large bilateral ischemic infarct of the pons. Computed tomography angiography did not demonstrate other large vessel disease or structural vascular abnormalities. Unfortunately, the patient was out of the time window for any acute stroke interventions. A hypercoagulable workup was performed but results were unrevealing and hematocrit was normal. Further investigation with transthoracic echocardiogram, transesophageal echocardiogram, and telemetry were negative for thrombus, patent foramen ovale, and atrial fibrillation.
Conclusions:
Acute ischemic stroke may be an under recognized complication of testosterone therapy in transgender males independent of degree of erythrocytosis. Further research is needed to establish a safety profile of testosterone therapy in this understudied population.
